This checklist is what I walk through for every purchase, whether it's a $400 spare part or a $40,000 imaging system. Eight steps. If you're a clinic administrator, practice manager, or the person who somehow got stuck with procurement at a small-to-mid-sized facility, this is for you.
Step 1: Start with the Clinical Need, Not a Brand Name
When a department head says “we need a new ICU monitor,” your instinct is to start Googling ICU monitors. Stop. Ask what it actually needs to do first.
Questions I ask every time:
- What parameters matter? ECG, SpO2, NIBP, EtCO2, temperature—or all five?
- Where will it be used? ICU, step-down, ED, or multiple units?
- Do you need central station integration, or will standalone units work?
- How many units are we talking about?
I once had two departments request “an ICU monitor” in the same month. The ICU team needed full hemodynamic monitoring with central integration—about $12,000 per unit. The rehab department needed a transport monitor to check vitals during patient transfers—about $3,500. Same words, completely different devices. Your first job is to understand the need, not to jump to a model number.
Step 2: Write Down the Non-Negotiables Before You Look at Options
Once the clinical need is clear, turn it into a spec sheet. Separate “must have” from “nice to have” from “we will literally never use this.” Then get the clinicians who will use it to sign off.
When we bought our first dental X-ray machine, I assumed “digital” meant it would work with any imaging software. It didn't. Our practice used a sensor system that required a specific software bridge, and the machine we ordered didn't support it. Fixing that cost $1,400 and about three weeks of waiting—or rather, four, counting the back-and-forth with the vendor.
A basic dental X-ray spec sheet should include:
- Intraoral, panoramic, or both?
- Sensor compatibility (which brand and software version)
- kVp range and exposure controls
- Mounting: portable, wall-mounted, or on the dental unit
- Compliance with state radiation safety rules
The signed spec sheet protects you too. When someone later says “this isn't what I meant,” you can point to the document they approved.
Step 3: Understand How the Equipment Actually Works
You don't need to be an engineer. But you need to understand the basic operating principle of anything you're buying, or you'll have no idea whether the specs make sense.
Take the autoclave. I get asked “how does an autoclave work?” more than any other question from clinic staff. Simple version: it uses saturated steam under pressure to sterilize instruments. Per CDC guidelines (cdc.gov), standard gravity-displacement cycles run at 121°C (250°F) for 30 minutes, or 134°C (273°F) for shorter cycles depending on the load.
Why does that matter when you're buying one? Because cycle time and chamber size dictate how many procedures your clinic can run per day. If you're doing back-to-back oral surgeries every morning, a small countertop autoclave with a 60-minute cycle will bottleneck your schedule. You need to know:
- Chamber size and how many instrument trays fit per cycle
- Cycle duration for wrapped vs. unwrapped loads
- Water source type—distilled, demineralized, or tap
- What biological/chemical indicator tests you'll need to run
Same logic applies to ICU monitors. Modular monitors let you swap parameter modules in as needs change, but they cost more upfront. Fixed-configuration monitors are cheaper and simpler. That one distinction will shape which questions you ask every vendor.
Step 4: Vet Vendors on Transparency, Not Just Price
I've fallen for the cheap-quote trap more than once. In 2021, I picked a vendor whose bid was $800 lower than the next one for a patient monitor. The device arrived without the wall-mount bracket. Batteries weren't included either. Neither was the service contract I assumed came standard. The add-ons totaled $1,400.
I've learned to ask two questions before any price discussion:
“What's NOT included in this price?”
“Are there any fees that could come up during installation or the first year?”
If a vendor hesitates or says “let's sort that out later,” that's a red flag. The lowest quote usually isn't the lowest once you add everything back in. From the outside, a low bid looks like efficiency. The reality is some costs are just hidden or deferred. The vendor who lists all fees upfront—even if the total looks higher—costs less in the end. That pattern has held across 200+ orders, give or take.
Step 5: Calculate the 5-Year Total Cost of Ownership
The purchase price is maybe a third of what a device costs over its useful life. Maybe less.
For an ICU monitor, remember to budget for:
- Mounting arms, extra cables, and a battery replacement around year 3 (usually $300–600 per monitor)
- Annual calibration or preventive maintenance ($150–400 per device per year, depending on your region)
- Central station software licenses, if you need integration
- Staff training—online is cheaper, but on-site training actually gets used
For a dental X-ray machine, the digital sensor is a consumable. A single intraoral digital sensor runs about $4,500–$7,000 as of February 2025 (based on quotes from two suppliers I work with; verify current pricing). If the machine only works with sensor technology that's being phased out, walk away.
For an autoclave, add the recurring cost of biological indicators, water quality tests, and annual certification—a few hundred dollars per year, depending on your jurisdiction.
When you compare vendor quotes, use a 5-year horizon. That's been standard in healthcare capital planning for decades, and it's the only fair way to compare upfront price vs. recurring costs.
Step 6: Verify Certifications and After-Sales Support
Don't buy anything in your country without checking regulatory status. In the US, most medical devices require FDA clearance—typically a 510(k) Premarket Notification—or Premarket Approval. You can verify a device's clearance number at accessdata.fda.gov. No clearance number? Move on.
In the EU, devices need CE marking under the Medical Device Regulation (MDR). In other regions, check your local health authority's list of approved devices.
Then ask the support questions:
- What's the response time for our region?
- Is there a service tech in our city, or does a part have to ship from overseas?
- What does the warranty exclude? They all exclude something.
Support looks different for certain equipment. Take the Sunrise Medical Zippie Iris—a lightweight pediatric wheelchair we order for our rehab unit. The questions there are about parts: wheel locks, tilt mechanisms, seating components. How fast can you get them? Can a local dealer repair the chair, or does it have to go back to the manufacturer? For equipment that kids use daily, a two-month parts delay is unacceptable.
Step 7: Confirm Delivery, Installation, and Training Before You Sign
Classic newbie mistake—I made it in my first year. I ordered an ICU monitor and assumed installation was included. It wasn't. We paid $380 extra and lost a week waiting for a technician to mount it properly on the wall arm.
Before signing, get these answers in writing:
- Who receives the delivery? If it's a heavy panoramic X-ray unit, is there a loading dock or elevator access?
- Who installs it—vendor, manufacturer, or a third-party tech? And does “installation” include mounting, or just placing it in the room?
- Who trains your staff, and how long is the training session? Is it included?
- Are all software features included, or are some locked behind an upgrade key?
That last question cost me personally. I assumed “standard configuration” included the full software set. Didn't verify. Turned out the ECG interpretation module was a paid upgrade—$4,500 extra. Learned never to assume the proof represents the final product after that.
Step 8: Set Up a Records System (and Actually Use It)
This is the step most buyers skip, and the one I'm most grateful I started. For every piece of equipment, keep track of:
- Serial number, model, purchase date, warranty end date
- Vendor contact—a specific person, not the general sales line
- Service history: every repair, calibration, inspection, and parts replacement
- Training log for staff who operate the device

Three Mistakes I Still See Buyers Make
1. Assuming “standard” means the same thing to every vendor
It doesn't. Make each vendor define what “standard installation,” “standard configuration,” and “standard training” include—in writing. I learned this after a $4,500 ECG module surprise.
2. Asking “what's the price?” before asking “what's NOT included?”
The most valuable question in procurement is the one that feels awkward to ask. Good vendors appreciate it because it prevents disputes later. Bad vendors get uncomfortable—that's useful too.
3. Underestimating downtime
The cheapest quote isn't cheap if the machine sits broken for six weeks waiting on a part. Before you purchase, ask about service response time in your area. That applies to autoclaves, monitors, X-ray machines, and wheelchairs alike.
This process has served me well since 2020, and I still revise it about once a year. If I remember correctly, every revision has removed an assumption and added a verification step instead. Nobody gets procurement right on day one—the goal is to avoid making the same mistake twice.
